Enhanced Care Coordination: Helping Children, Youth, and Families Impacted by Substance Misuse

PRESENTATION:This 6-hour course will provide participants with an introduction to person-centered, enhanced care coordination (ECC), with a specific emphasis on families impacted by substance misuse and co-occurring challenges.  Unlike case management, ECC is a facilitated process where the coordinator partners with clients based on foundational principles such as person-centeredness, strengths-based care, and resilience.

Using a mix of lecture, demonstration, video, and small group activities, participants will learn how to: 

  • Use multiple techniques such as active listening, graphic facilitation, and strengths assessments to engage the child, youth, and family/caregiver. 
  • Work effectively with natural family support networks and peer supporters. 
  • Participate as an effective member of and facilitate interdisciplinary child and family teams. 
  • Understand and address the unique developmental needs young children and youth who have experienced trauma and who have emotional or behavioral challenges. 
  • Demonstrate cultural humility and understand structural inequality and implicit bias.
