The first webinar in this series will focus on care coordination, an approach utilized at the individual level that involves strong cross-collaboration and shared responsibility across clinical teams and providers to improve upon and provide whole person care. This session will explore the role of the care coordinator, identify the various functions of effective care management and discuss how organizations can plan for change to improve their care coordination, with special focus on care coordination within Certified Community Behavioral Health Clinic (CCBHC) settings during the ongoing COVID-19 pandemic.
